Colorectal Cancer
 Imran, Pakistan

【Patient profile】  Imran, 55 years old, General Manager of a textile company in Karachi, Pakistan

【Predicaments before coming to China】

Sigmoid colon cancer with simultaneous multiple liver metastases (initially unresectable). Pakistan only offers basic FOLFOX chemotherapy and lacks a complex multidisciplinary translational therapy system for tumors. Advanced radical resection techniques such as portal vein embolization (PVE) and combined liver segmentation and staging surgery (ALPPS) are not feasible, making radical tumor resection difficult.

【The decision-making process of choosing Trisana Medguide】

Recommended by the Karachi Chinese Enterprises Association, Trisana Medguide arranged:

- Urdu/English medical record translation, data standardization and organization

- Connect with the Colorectal Cancer MDT Center of Fudan University Cancer Hospital to conduct multidisciplinary team remote consultations.

- Customized, individualized comprehensive treatment plans including "conversion therapy + liver pretreatment + staged radical surgery" and medical planning in China.

【A complete record of my medical treatment in China】

| Stage | Service Content |

| First trip to China (14 days) | Complete comprehensive and precise staging assessment of the whole body and liver reserve function assessment; performed portal vein right branch embolization (PVE) surgery to induce residual liver hyperplasia; simultaneously carried out two cycles of standardized conversion chemotherapy; after stabilizing the condition, returned to Pakistan to continue phased chemotherapy; provided remote follow-up guidance throughout the process. |

| Interval period (8 weeks) | Continuous remote follow-up management, regular monitoring of tumor markers, chemotherapy tolerance, and changes in liver volume; follow-up examination showed that the left hepatic volume increased from 28% to 38%, meeting the safety criteria for surgery, and assessment indicated that radical surgery could be performed. |

| Second trip to China (21 days) | An individualized staging surgical strategy was developed based on the comprehensive assessment of the MDT. Radical resection of the sigmoid colon cancer was performed first, followed by elective ALPPS step one (combined liver division and portal vein ligation) after the postoperative recovery period. Once the remaining liver hyperplasia reached the target and the patient's overall condition permitted, ALPPS stage two surgery was performed, ultimately achieving complete resection of the lesion at R0.

(Note: This combined surgical procedure is a highly complex and challenging operation that requires rigorous preoperative evaluation. The specific staged treatment plan will be dynamically adjusted based on the patient's intraoperative and postoperative recovery.)
